New patient paperwork can now be accessed via the Jane portal. When you schedule your appointment, you will receive an email that will invite you to create an account with Jane software. Patients have access to a secure portal through Jane called "My Account". This account can be set up when you complete the online booking process or when you receive your welcome email. You also have access to your appointment history and upcoming appointments. You now have the ability to pay patient balances securely through the Jane portal using Stripe or at our front desk (Visa/MC/AMEX/Discover/GooglePay/ApplePay). For patients without email access, paper copies of our intake paperwork are available for download and printable below.
